In addition we will not collect any additional information other than that which will facilitate medical care in an emergency.

With whom the information may be shared Information about children under the age of 13 whose parents have enrolled online will be used by Emergency Personnel to notify doctors, hospitals, emergency rescue teams, and family members that the child was found carrying an Access My Records, LLC AccessID^SM Card or AccessID^SM Key Tag. Their condition, medications, and dosages, and any other health issues will be conveyed to the above parties. Personal and health information will also be conveyed to the responder for the purposes of treating the child in emergencies situations.

Security procedures that are in place to protect the loss, misuse, or alteration of information.
We are required by law to take reasonable security measures to protect personal information about children. Registration information submitted by a child's parent cannot be modified except by the parent/guardian themselves that enrolled the child. All such information is contained behind secured networks and is only accessible by a limited number of employees and contractors who have special access rights to such systems. All contractors have signed confidentiality agreements with us. All credit card information supplied is transmitted via Secure Socket Layer (SSL) technology and then encrypted into our databases to be used and accessed only as described in this policy.

How you can review and delete any information collected from your child?
The legal Parent/guardian who enrolled their child with Access My Records, LLC. online may also review or modify the information about their children within their electronic medical health record. Before disclosing information about a child, we may request that the parent provide reasonable information about the parent for identification purposes, such as the parent's name, address, phone number, credit card number, or other information.

How you can opt not to share personal information collected from your child with 3rd parties, if you choose.
Children under the age of 13 will never be asked to consent to disclosure to third parties, and their personal information will not be disclosed to any third parties other than the agencies listed above. A parent's consent to our collection and use of personal information about a child will never be treated as a consent to disclosure of that information to third parties.

If a parent wishes to terminate the child's membership at anytime this can be done by calling 561-443-5508 and requesting the appropriate form for termination.
